Water fasting is one of the oldest practices within Natural Hygiene. It removes the work of digestion for a period of time and allows the body to rely on stored fuel — but it also creates significant physiological changes that deserve to be understood properly.

A water-only fast means consuming water and no food or caloric drinks. As the fast continues, the body moves through a predictable series of metabolic changes: stored glycogen falls, insulin drops, fat use increases and ketones become a larger source of fuel.

Natural Hygiene has traditionally viewed fasting as a period of physiological rest. Modern research confirms that fasting is far from a passive state: hormones, fuel use, blood pressure, body weight, inflammatory signalling and cellular metabolism can all change.

That does not mean longer is always better, or that every claim made about fasting has been proven. The effects depend on the person, the length of the fast, their nutritional status, medications, health conditions and how the fast is broken.

What Is Water Fasting?

During a true water-only fast, no calories are consumed. The body therefore has to meet its energy needs from stored glycogen, body fat and, to a lesser extent, body protein.

This is different from juice fasting, broth fasting, time-restricted eating or intermittent fasting. Those approaches still provide some energy or include daily eating periods. Water fasting is a complete temporary absence of food.

What Happens in the Body During a Water Fast?

1. Glycogen Begins to Fall

After food stops, the body initially relies heavily on glucose and stored glycogen, particularly from the liver. Glycogen also holds water, which is one reason body weight can drop quickly in the early stages of fasting.

2. Insulin Falls and Fat Use Increases

As circulating glucose and insulin fall, stored fat becomes increasingly important. Fatty acids are released from adipose tissue and used for energy.

3. Ketone Production Rises

The liver converts some fatty acids into ketone bodies, including beta-hydroxybutyrate. These can be used by the brain and other tissues, helping reduce the body’s dependence on glucose.

This metabolic shift is one of the best-established effects of fasting. It develops progressively rather than at one exact hour, so claims that ketosis or autophagy suddenly “switch on” at a precise time should be treated cautiously.

4. Water and Sodium Balance Change

Falling insulin alters kidney handling of sodium and water. This can contribute to the rapid early loss of body water and may also help explain why some people experience light-headedness, lower blood pressure or weakness when standing.

5. The Body Continues to Use Some Protein

Fasting is often described as though the body burns only fat once ketosis begins. That is not quite correct. Protein turnover continues and some amino acids are used for glucose production and other essential functions.

A 2024 review of prolonged-fasting trials found reductions in both fat mass and lean mass. The exact proportion varies between studies and methods of measurement, but it is one reason prolonged fasting should not be treated casually.

Potential Benefits of Water Fasting

The strongest human evidence does not support every claim commonly made online, but several effects have been repeatedly observed.

Weight Loss

Weight falls during a water fast because stored glycogen, water, fat and some lean tissue are being used. Reviews of prolonged fasting lasting roughly 5–20 days have reported weight losses of around 2–10% of starting body weight.

Some of that weight returns during refeeding as glycogen and water are restored. Long-term results depend far more on how somebody lives after the fast than on the fast alone.

Blood Pressure

Several supervised prolonged-fasting studies have reported reductions in systolic and diastolic blood pressure. This is particularly relevant for people starting with elevated readings, although medication use can make fasting more complex and should not be adjusted casually.

Glucose and Insulin

Fasting lowers insulin while food is absent, and some studies have found improvements in fasting glucose, insulin and measures of insulin resistance in people without diabetes.

The picture is less simple in people with diabetes. A major review found that improvements seen in people with normal glucose regulation did not consistently appear in participants with type 1 or type 2 diabetes. Diabetes medication can also make fasting unsafe without appropriate supervision.

Metabolic Flexibility

Water fasting forces the body to move away from continual incoming carbohydrate and towards stored fuel. Ketone production rises and fat oxidation becomes dominant. From a Natural Hygiene perspective, this is one reason fasting is viewed as a period in which energy normally devoted to digestion is no longer being spent on processing food.

Digestive Rest

One of Natural Hygiene’s oldest descriptions of fasting is simply “rest from digestion”. When no food enters the digestive tract, there is no new meal to mechanically and chemically process.

That does not prove that every digestive problem will resolve through fasting, but the absence of incoming food clearly changes the workload of the gastrointestinal system.

What About Autophagy?

Autophagy is the cell’s recycling and quality-control system. It helps remove damaged cellular components and reuse their building blocks.

Fasting and nutrient deprivation can influence autophagy pathways in laboratory and animal research. The difficulty is that autophagy is not something we can easily measure throughout the human body in real time.

So the claim that “autophagy begins at 18 hours” or “peaks at 72 hours” goes beyond what current human research can establish. It is more accurate to say that fasting changes nutrient-sensing pathways associated with cellular maintenance, while the exact timing and magnitude vary between tissues and individuals.

The Natural Hygiene View of Fasting

Natural Hygiene does not see fasting primarily as a weight-loss trick. Traditionally, fasting is used as a period of rest when the body is no longer required to process incoming food.

The wider philosophy matters. A fast is not intended to compensate indefinitely for poor sleep, chronic stress, smoking, alcohol, highly processed food or an unsustainable lifestyle. The conditions after the fast matter just as much as the fast itself.

This is also why Natural Hygiene distinguishes fasting from starvation. A voluntary fast is undertaken while the body still has adequate stored reserves and is stopped before those reserves are exhausted.

Common Effects During a Water Fast

People can experience very different fasting responses. In supervised studies, commonly reported effects have included:

  • hunger
  • fatigue or weakness
  • headache
  • dizziness or light-headedness
  • dry mouth
  • changes in sleep
  • nausea
  • lower blood pressure
  • feeling cold
  • changes in bowel activity
  • changes in breath or taste as ketones rise

Mild symptoms are common, but not every symptom during a fast should automatically be interpreted as a beneficial “healing crisis”. Persistent vomiting, fainting, severe weakness, confusion, arrhythmia, significant dehydration or other worrying symptoms need proper assessment.

What Are the Risks?

Water fasting can create meaningful physiological stress as well as metabolic adaptation.

  • Low blood pressure and dizziness: fluid and sodium changes can contribute to orthostatic symptoms.
  • Electrolyte disturbance: prolonged fasting can alter potassium, sodium and other electrolytes.
  • Loss of lean mass: prolonged fasting reduces body fat but can also reduce lean tissue.
  • Medication interactions: insulin, blood-pressure medicines, diuretics and other drugs may behave differently when food intake stops.
  • Dehydration or overhydration: drinking too little or forcing excessive amounts of water can both cause problems.
  • Arrhythmias: abnormal heart rhythms have been reported in fasting studies, particularly when electrolyte balance is disturbed.
  • Refeeding syndrome: after prolonged fasting or significant malnutrition, restarting calories can trigger dangerous shifts in phosphate, potassium, magnesium, thiamine and fluid balance.

A Newer Finding: Fasting Does Not Always Lower Inflammation

Fasting is frequently promoted online as automatically “anti-inflammatory”, but newer human research shows the biology may be more complicated.

A 2025 medically supervised water-only fasting study found expected rises in ketones and reductions in body weight, but it also reported increases in several inflammatory and platelet-activation signals during prolonged fasting.

This does not mean fasting is inherently inflammatory or harmful. It does show why fasting should not be reduced to slogans. The body is undergoing a major adaptation, and different biological systems may move in different directions at the same time.

Who Should Not Undertake Prolonged Unsupervised Water Fasting?

Longer water fasts require more caution than skipping a meal or fasting for a day. Prolonged unsupervised fasting is generally inappropriate for people who are:

  • pregnant or breastfeeding
  • children or adolescents
  • underweight or significantly malnourished
  • living with an active eating disorder or a strong history of restrictive eating
  • taking insulin or medicines that can cause low blood glucose
  • taking medications where food intake or fluid balance materially affects dosing
  • living with serious kidney, liver or heart disease
  • frail or medically unstable
  • recovering from major surgery or serious acute illness

The exact decision depends on the individual rather than a simple list. The longer the proposed fast, the more important screening, monitoring and an appropriate refeeding plan become.

How Much Water Should You Drink?

There is no universal litre target that is appropriate for everyone. Water requirements vary with body size, temperature, activity, kidney function and how much fluid would normally come from food.

Natural Hygiene traditionally encourages drinking according to thirst rather than forcing large quantities of water simply because you are fasting. Excessive water intake can dilute sodium, while inadequate intake can contribute to dehydration.

Can You Exercise During a Water Fast?

Gentle movement may feel comfortable during a short fast, but hard training is a different matter. During prolonged fasting the body is operating without incoming energy and may also be losing fluid, sodium and lean tissue.

Natural Hygiene generally places more emphasis on rest during therapeutic fasting than on trying to maintain normal training volume. If you feel weak, dizzy or unusually depleted, forcing exercise works against that principle.

How Should a Water Fast Be Broken?

Refeeding is part of the fast. At Natural Body Intelligence we use a fruit-based Natural Hygiene refeed: fresh, ripe fruit in small amounts initially, with the quantity increased gradually according to the length of the fast and the person’s condition.

A one-day fast needs far less refeeding than a ten-day fast. After prolonged fasting, nutritional status and refeeding-syndrome risk matter as well as the type of food used.

Is Water Fasting Safe?

The best safety data come from medically supervised programmes, not people fasting alone at home. A large chart review of 768 medically supervised water-only fasting visits found that most recorded adverse events were mild to moderate, although serious events did occur.

That distinction matters. Evidence that a structured protocol can be carried out with a relatively low rate of serious complications in a monitored setting is not evidence that any person can safely undertake an open-ended water fast without screening or supervision.

What Does the Research Actually Tell Us?

Human research supports several real physiological effects of water fasting: ketosis rises, body weight falls, blood pressure often falls, and glucose-insulin metabolism changes.

What is much less certain is how durable these changes are, how long an optimal fast should be, which conditions genuinely benefit, and whether repeated prolonged fasting improves long-term health outcomes.

A 2024 review found that many metabolic improvements seen immediately after prolonged fasting were no longer evident several months later. That makes the post-fast lifestyle essential. Fasting can create a physiological reset point, but what happens afterwards determines whether change lasts.

The Natural Hygiene Bottom Line

Water fasting is not simply “not eating”. It is a distinct metabolic state in which the body changes fuel sources, hormone signalling, fluid handling and tissue use.

Natural Hygiene has used fasting for generations because it removes the constant demand of digestion and allows a period of deep rest. Modern research confirms many of the metabolic changes, while also showing that fasting has limitations and genuine risks.

The useful question is not whether fasting is universally good or bad. It is whether it is appropriate for the individual, how long it is continued, how the person responds, and what conditions they return to afterwards.
