What if some of the things we call “disease” are not simply evidence that the body has stopped working properly?
That question sits at the heart of one of the most important — and now largely forgotten — concepts in Herbert M. Shelton’s Natural Hygiene writings: Orthopathy.
Shelton devoted an entire volume of The Hygienic System to the subject in 1939. Its chapters examined fever, inflammation, crises, pathology, enervation, recovery and the concept he later described more precisely as Biogony.
What does Orthopathy mean?
The word is built around the idea of right or correct suffering.
That sounds strange until we understand what the early Natural Hygienists were trying to describe.
They challenged the assumption that every unpleasant symptom must represent an enemy attacking the body or a malfunction that needs to be stopped. Instead, they asked whether some symptoms might arise from the body’s own physiological responses to harmful conditions.
Orthopathy was therefore not simply another word for disease. It was a framework for asking what the living organism is doing during disease.
The crucial distinction: activity versus damage
This distinction is essential to understanding Shelton.
He did not argue that every phenomenon labelled disease was beneficial. He explicitly distinguished active physiological responses from structural damage and degeneration.
Blindness, paralysis, tissue destruction, degeneration and loss of function are not the same kind of phenomenon as fever, vomiting, diarrhoea or inflammation.
Yet historically they could all be placed beneath the enormous umbrella word disease.
Shelton believed this confused two very different things:
- Pathology — damage, deterioration or abnormal structural condition.
- Physiological response — activity performed by the living organism in response to circumstances.
This problem eventually led him to coin the term Biogony for the active, defensive or remedial processes that he believed were being wrongly grouped together with pathology.
Why fever mattered so much to Shelton
Fever provides an obvious example of the distinction Shelton was trying to make.
A fever is not simply the body becoming randomly hot. Body temperature is actively regulated, and during fever the physiological temperature set-point changes. Shivering, altered blood flow and changes in heat production can then raise body temperature towards that new set-point.
Modern physiology therefore recognises fever as a regulated biological response rather than merely uncontrolled overheating.
Shelton went considerably further in his interpretation, seeing fever as part of the body’s remedial effort. Whether or not every part of his historical explanation is accepted today, his underlying question remains useful: before suppressing a symptom, should we first understand what physiological process is producing it?
The same question applies to inflammation
Inflammation is another process often spoken about as though it were itself an external enemy.
Yet inflammation involves coordinated changes in blood flow, vascular permeability, signalling and cellular activity. It is something the body does.
That does not mean inflammation can never become excessive, persistent or damaging. It means the process cannot be understood properly if we only describe its outward discomfort.
Orthopathy asks us to investigate the purpose, cause and context of the response.
Disease as redirected physiology
Shelton repeatedly argued that the body does not acquire an entirely new set of powers when it becomes ill.
The same organism that digests, circulates, eliminates, regulates temperature and repairs tissue in health remains the organism performing physiological activity during illness.
From this perspective, many symptoms can be viewed as normal physiological mechanisms operating under abnormal conditions.
This connects directly with Bionomy — the idea that living functions operate according to biological laws.
Cause comes before symptoms
Orthopathy shifts attention away from the symptom alone and towards the circumstances that preceded it.
If the conditions creating a burden remain unchanged, merely altering the outward expression does not necessarily address why the response arose.
This is one reason Natural Hygiene places such importance on the conditions of life: sleep, rest, food, movement, fresh air, sunlight, water, emotional state and the avoidance of unnecessary harmful exposures.
That wider relationship between organism and environment is the subject Shelton called Orthobionomics.
Enervation: when available functional capacity falls
Another concept repeatedly appearing in Shelton’s Orthopathy was enervation.
Natural Hygienists used this term for a state in which functional or nervous energy was depleted by factors such as overwork, insufficient rest, emotional strain, stimulation and unhealthy living habits.
The historical terminology is different from modern physiological language, but the underlying observation — that organisms have finite capacity and that chronic demands affect recovery — deserves examination in its own right.

Orthopathy and the healing crisis
Orthopathy also helps explain why Natural Hygiene developed its distinctive interpretation of the healing crisis.
When conditions change — particularly during rest, dietary change or fasting — symptoms can sometimes change too. Early Hygienists interpreted some temporary increases in physiological activity as part of the body’s adjustment to altered conditions.
That does not mean every symptom experienced during fasting or lifestyle change should automatically be labelled a healing crisis. Symptoms can also signal dehydration, deficiencies, injury, disease progression or other problems requiring attention.
The useful lesson from Orthopathy is not to assume. It is to understand what is happening before deciding what a symptom means.
The body as a coordinated whole
Orthopathy also fits the Natural Hygiene view that the body should not be understood as a collection of independent parts.
The nervous, digestive, endocrine, circulatory, lymphatic and other body systems continuously interact.
A response visible in one area may therefore reflect changes occurring throughout the organism.
This whole-body perspective is fundamental to Natural Body Intelligence: the symptom is information, but it is not necessarily the whole story.
Orthopathy versus the idea of “fighting disease”
One of the biggest conceptual differences is language.
We commonly speak of fighting disease, battling symptoms and defeating illness.
Orthopathy asks whether, in some circumstances, that framing accidentally places the body itself on the opposing side.
If fever, inflammation, increased mucus production, coughing, vomiting or diarrhoea involve physiological actions generated by the body, the first question becomes not simply “How do we stop this?” but “Why is the body producing this response?”
Again, this does not mean every response should be allowed to continue without limits or that serious symptoms should be ignored. It means understanding physiology should come before simplistic assumptions about what the symptom represents.
Why Orthopathy still matters
The word itself may never return to everyday language.
But the distinction behind it remains powerful.
Is what we are observing:
- the cause of a problem?
- the damage created by a problem?
- a physiological response to the problem?
- an adaptation?
- an attempt at compensation?
- or some combination of these?
Those are very different possibilities.
Using one word — disease — for all of them can hide those distinctions.
The Natural Body Intelligence perspective
At Natural Body Intelligence, this distinction is central to the way we explore health.
The body is continuously responding to its circumstances. It regulates, compensates, conserves, repairs, eliminates and adapts.
Symptoms therefore need context.
Rather than automatically assuming that every uncomfortable response means the body has failed, we ask what changed, what preceded it, what biological process is occurring and what conditions the body currently has available to it.
This is also why understanding Natural Hygiene requires more than learning about diet or fasting. It requires a different way of thinking about the relationship between life, environment, symptoms and recovery.
The forgotten language of Natural Hygiene
Orthopathy now joins the other concepts in our growing exploration of Natural Hygiene’s forgotten vocabulary:

- Bionomy — the study of the laws and functions of life.
- Orthobionomics — adapting the conditions of life to biological needs.
- Orthotrophy — correct nourishment and feeding.
- Orthokinesiology — correct movement and exercise.
- Orthogenetics — reproduction, parenthood and development within Shelton’s Hygienic System.

The terminology belongs to the history of Natural Hygiene, but the questions Shelton was asking — what is damage, what is response, what is adaptation, and what conditions support recovery — remain central to understanding the philosophy today.
